Request a prenatal consultation
Currently, all of our physicians are accepting newborn patients.
If you are interested in meeting with one of our doctors to see if Children's Clinic is right for you and your newborn, please complete the Prenatal Request Form and we'll get back to you as soon as possible.
Request to transfer
If you wish to transfer your children to Children's Clinic, the process is easy. Drs. Atkinson, Calfee, Kreusser, McCormick, Pingel, and Sonderman are currently accepting transfer patients. Please complete the Patient Transfer Form to begin the process. If you have more than one child you'd like to transfer, please fill out a form for each child. We advise that you make sure that we are participating provider on your insurance before starting. Please note that we do not accept any state insurance plans.
Before scheduling a new patient visit, we ask for the child's full medical record to be mailed or brought into the office (we are unable to copy records and ask that records not be faxed). Once the records are reviewed by your chosen pediatrician, a staff member will contact you to schedule an appointment. Please use this form to assist you in transferring records to us. Release form
Please note, children are considered "established patients" once they have been seen in the office at a scheduled appointment. New patients will be unable to be seen during our walk-in time or our Saturday office hours until they are "established patients."
Once you have an appointment scheduled, please download and complete the following forms to bring to your first appointment.
